Sr Nurse Practitioner
Job Description
Clinical Leadership: • Provide expert clinical care as a Nurse Practitioner, including assessment, diagnosis, treatment, and follow-up care for patients in accordance with organizational protocols. • Serve as a clinical resource for Advanced Practice Providers, ensuring adherence to evidence-based practices. • Assist the Chief Medical Officer develop and implement clinical guidelines and protocols to standardize care delivery and improve patient outcomes.
Program Development: • Design and implement initiatives to optimize the integration and utilization of Advanced Practice Providers within care teams. • Identify and lead opportunities to enhance service delivery, improve patient outcomes, and address health disparities. • Collaborate with interdisciplinary teams to develop and expand programs, such as behavioral health integration and harm reduction initiatives. Operational Management: • Develop schedules, ensure adequate staffing, and optimize workflows to balance patient access and provider capacity. • Monitor and evaluate the performance of MA’s, providing regular feedback, coaching, and professional development opportunities. Quality Improvement: • Lead quality improvement initiatives focused on clinical outcomes, patient satisfaction, and operational efficiency. • Analyze clinical and operational data to identify trends, gaps, and opportunities for improvement. • Ensure compliance with regulatory standards and organizational policies.

PHC provides medical and behavioral health services at its clinic in the Opportunity Center in Palo Alto, and community-based, intensive case management, mental health, and housing services through the New Directions Program.
